Welcome to the CB&W Family
Step 1: Pay your deposit
This deposit reserves your spot in your birth month or on our waiting list in order of receipt and will be credited towards services provided (medical and/or non-medical). In order to schedule your first prenatal appointment, this deposit must be paid. We will reach out once we receive your payment to get you on the calendar with one of our midwives. Once your deposit is paid, you will receive a link to ClientCare.
Step 2: Client Care
Once you receive your link to set up your ClientCare account please do so! Check your Spam and Promotions folder for the invite if you don’t see it in your main inbox. If you can not locate your invite please email us at info@cobirthwell.com
Client care is where you will receive emails from your midwife team, appointment reminders, and have 24/7 access to your medical records.
Step 3: Fill Out Records Request
If you have been seen at a previous provider for this pregnancy we will need to have your records prior to booking your first appointment. Please complete the Records Request found in the “Forms and Records” Section of ClientCare.
A call to your current provider that a request is on the way can expedite the process.
Step 4: Insurance and Finances
Provide your insurance card and identification card by uploading to ClientCare. An estimate will be completed based on the information provided in ClientCare and you will receive a ClientCare message when it’s available. If you’re in-network an estimate will be completed based on your in-network deductible. If you’re self pay, your cost for routine global services will be approximately $7600.
Complete the payment plan form provided in ClientCare and ensure all financial policies/new client consents are signed

Step 5: Sign Consents and Disclosures
Once in your ClientCare account, please access the tab that states “Forms and Records” . Here you will find the needed consents and disclosures that will need to be signed and completed prior to your first appointment. Step 6: Fill Out Health History
Once in your ClientCare account, please access the tab that states “Health History” . Please fill out every portion to the best of your ability so that our providers can prepare best for your appointment.
Step 7: Sign Up for a Doula
First time parents are NOT required to have a doula at their birth. However, we encourage you to understand and ask us about the benefits of having a doula. A great place to learn more is on our Doula Page.
Step 8: Classes and Education
Return to the main client page to find our education offerings and sign up for classes. Reach out to us if you have any questions on how or what to sign up for.
